Job Summary: The Service Coordinator works alongside a team of advocates to help people with Intellectual and Developmental Disabilities (IDD) who are pursuing access to community services and support.
The focus of this role is to partner with individuals with IDD and involved loved ones - to aid access to services and safeguard the continuity of care. Service Coordinators apply a person-centered approach and perform a set of four service coordination elements - assessment, service planning/coordination, monitoring, and crisis prevention/management. Efforts commonly include working within a multidisciplinary team consisting of internal or external community partners.
Service Coordinators within the IDD division of Integral Care support the efforts of Intake, Enrollment, General Revenue, or Medicaid waiver programs - all of which work to some degree with person-directed plans and electronic health record systems upon training. In addition, Service Coordinators may also work with individuals with IDD residing in Medicaid Certified Nursing Facilities or the Austin State Supported Living Center seeking to move to homes in the community.
Qualified Mental Health Professionals (QMHP) embrace trauma sensitive and informed practices, provide suicide safer care, and foster a supportive and welcoming environment that promotes racial and health equity. QMHPs operate in a dynamic environment, collaborate with interdisciplinary teams and diverse community members, and develop extensive knowledge of community resources. Services can be both office and community based and may require the use of a personal automobile. Individuals served are diagnosed with IDD but may also present with co-occurring mental illness, substance use disorders, and/or chronic health conditions.
Work is performed under general supervision with moderate latitude for the use of initiative and independent judgment.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Salary: $22.35 - $23.47 an hour, based on education.
