Qualified Mental Health Professional Team Lead (QMHP)

Job Summary: This QMHP Team Lead position will work to serve clients in the new crisis residential Therapeutic Diversion Program (TDP) at 15th St. The primary duties include providing clinical support and intensive case management for clients working on ongoing stabilization with therapeutic interventions, relapse prevention services, and facilitating transition and linkage for continuity of care to individuals experiencing challenges with mental health and/or substance abuse, homelessness and criminal justice involvement within the community. The QMHP Team Lead will assist with guiding and coordinating clients' treatment planning to include their desired goals and identified tasks for client self-care through lived experience and applicable skills sets. The QMHP Team Lead will be working alongside the multi-disciplinary team staff to comply and adhere to contractual criteria, agency guidelines and program requirements. The QMHP Team Lead will work in close collaborations TDP Practice Manager (PM) as well as other personnel and partners to continuously assess, develop and refine the services delivery system to assist clients' success, offering optimal resources with compassion and using evidence-based practices. TDP is designed to offer longer term stabilization based on the clients' needs and goals while residing at the 15th St. unit. The QMHP, in conjunction with the PM, will engage with other community supporting stakeholders to promote cohesive, comprehensive, quality and effective therapeutic diversion and deflection for individuals' to be able to establish a productive and positive foundation for sustainable, independent and healthier living .

The QMHP Team Lead will provide outreach and engagement and ongoing services to clients admitted to the Therapeutic Diversion Program. The QMHP Team Lead will assist individuals with navigation and accessing resources that promote a healthy lifestyle per the Social Determinants of Health. The QMPH Team Lead will work with individuals in developing skills necessary to manage symptoms, obtain/maintain services such as medical care, nutritional assistance, financial assistance, transportation, legal assistance, housing, or other resources. Individuals served at this level are assessed for crisis services as needed. The QMHP Team Lead position will be located in the community and in settings where individuals receive services and live. Interventions and services may be in the community or the office and may require the use of a personal vehicle. The QMHP Team Lead will represent Integral Care in the community providing outreach and education about services being offered. The QMHP Lead will promote communication and understanding between community members and providers, increase the use of health services in a community, improve adherence to health recommendations, and even decrease emergency services.

The QMHP Team Lead is responsible for leading the team's service delivery and ensuring the provision of recovery-focused, person-centered care with a trauma-informed perspective. The QMHP Lead operates in a dynamic environment, develops extensive knowledge of community resources, collaborates with interdisciplinary teams and cultivates a clinical skill-set by forging therapeutic relationships with diverse community members. The QMHP Lead participates in diagnostic evaluations and provides assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) to ensure optimal and personalized services. The QMHP Lead identifies and implements strategies for ongoing program development and achievement of unit performance measures and engages staff to ensure the highest quality of care and effective use of provider staff time and resources. This position is responsible for oversight of assigned program and personnel and may carry an individual case load. This position is responsible for supervision of staff within the assigned unit including, but not limited to, areas of client care, self-care, professional development, time management, performance review and feedback. The QMPH Team Lead will be working under a LPHA that provides ongoing evaluation and oversight of all assessments, diagnoses, and treatment.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. 

License:

Salary: $28.53 an hour plus an additional 10% crisis stipend.
