Job Summary: This position is part-time, and is community based. This position provide mileage reimbursement. Duties include completing surveys for services with clients in the community. The population served includes individuals 15-30 years of age who have experienced a first episode of psychosis.
The Qualified Mental Health Professional (QMHP) provides case management, advocacy and service coordination that may include: linkage to substance abuse treatment programs, HIV medical care and other social service providers in the community for HIV positive individuals who are living with mental health disorders, and/or current substance use, or a history of substance use with current risk factors for relapse. The position provides coordination of community resources to assist clients in accessing medical, social, educational, financial and other appropriate services that help the individual achieve an improved quality of life. Services may include: psychiatric rehabilitative skills training in symptom management and medication management, development of the skills necessary to obtain services, which include but are not limited to: medical care, dental care, food, clothing, utilities, financial assistance, transportation, legal assistance, housing and the coordination of these services to assist the client in obtaining and maintaining these services. This position may also provide counseling services in accordance to the position's licensure and accepted scope of practice.
The QMHP is also responsible for preliminary assessment and diagnosis of a client's presenting Axis I psychiatric and/or co-occurring disorder(s), development and implementation of a person centered care plan to provide services and treatment sought by the client, ongoing review of progress towards treatment, ongoing assessment, and coordination of appropriate services to facilitate each client's success in achieving and maintaining the maximum possible level of independent functioning. Also responsible for administering the HOPWA (Housing Opportunities for People with AIDS) Program in accordance with procedures and objectives set forth by the funding source. Staff in this position may also facilitate or co-facilitate psychoeducational groups. Work is performed under moderate supervision with moderate latitude for the use of judgment and initiative. Although staff members in this position are not typically LPHAs, staff members in this position are working under the supervision of the treating psychiatrist and a LPHA who provide ongoing evaluation and oversight of all assessment, diagnosis and treatment. At this level, assignments may
be of moderate complexity and are handled with considerable independence, and complicated cases receive closer clinical review and supervision. Work is performed under general supervision with moderate latitude for the use of initiative and independent judgment.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.
