Licensed Therapist (LPC, LMFT, LCSW) Job Summary: 

The LPHA, Continuity of Care Housing Navigator performs moderately complex coordination work, provides preliminary assessment of a client's presenting psychiatric and/or co-occurring disorder(s), develops a plan to provide linkage to services sought or needed by the client, and coordinates appropriate services to facilitate each client's success in achieving and maintaining the maximum possible level of independent functioning.  This position is community-based and provides short term services to adult clients admitted to/discharged from the local psychiatric hospitals, psychiatric respite centers, medical hospitals and jails in Travis County. The services provided will focus on linkage to resources and aftercare, decreasing re-hospitalization as well as linking to resources in the community for individuals experiencing homelessness. This position serves individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions utilizing a recovery based and trauma-informed perspective. Individuals must have been literally homeless the night before entering a hospital/jail/respite center and may not have resided in that facility for more than 90 days.  The services that Continuity of Care Housing Navigators provide require frequent coordination with external and internal providers is necessary to support individuals in achieving independent functioning.  Continuity of Care Housing Navigators provide coordination and discharge planning to assure that the individual is discharged to the least restrictive environment.  Services may include coordination to assist the client in obtaining or maintaining employment, housing, medical care, dental care, food, clothing, utilities, financial assistance, transportation, legal assistance, or any other basic human need. This position may also assist in connecting to ongoing support by completing intakes.

This position works under the supervision of the 3000 Oak Springs Program Manager who provides ongoing evaluation and oversight of all assessments, crisis intervention, caseloads and client coordination. This position may have additional duties as assigned.

Interventions and services are typically provided in the community and require the use of a personal automobile. 

Licensed Therapist (LPC, LMFT, LCSW) Education: 

Required:

  • Graduated from an accredited college or university with a Master's degree with a major specialization in Human/Behavioral/Social Services or related field.
  • Licensed Therapist (LPC, LMFT, LCSW) License:

Required:

  • Current and valid driver's license and a clean driving record.
  • Licensed Practitioner of the Healing Arts (LPHA), LPC, LCSW, Licensed Psychologist, or LMFT.

Licensed Therapist (LPC, LMFT, LCSW) Experience:

Required:

  • Experience performing mental health assessments, diagnosis, and treatment 

Hours: Full-Time; Monday-Friday, with flexibility. 

Salary: $55,602 - $63,616 annually, depending on licensed experience.
