Job Summary: Case Managers provide services to individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions utilizing a recovery based and trauma-informed perspective. Case Managers operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill set by forging therapeutic relationships with diverse community members. Interventions and services can be both office and community based and may require the use of a personal automobile and cell phone.
Case Managers work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes. Case Managers participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services. Case Managers work with individuals in developing skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as medical care, nutritional assistance, financial assistance, transportation, legal assistance and resources fulfilling any basic need. Case Managers, depending on unit assignment, may also facilitate psychosocial rehabilitative skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities. Individuals served at this level may be assessed for crisis services as needed.
Although staff members in this position are not typically LPHA's, staff members in this position are working under the supervision of a LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment.
This position may have additional duties associated with providing bilingual communication services and requires the
completion of a language competency test.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Salary: $23.25 - $24.41 an hour based on education.
