Description: The FACT model is an intensive, multi-disciplinary team-based intervention shown by research to reduce recidivism rates for people with serious mental illnesses and high risk for recidivism who cycle through the criminal justice system, and have a pattern of over-utilizing law-enforcement, jail, local emergency system, and hospital services. Clients served by FACT have been found to be incompetent to stand trial in the past year or are high utilizers of health care and the criminal justice systems for behaviors related to their illness. The individuals served often experience comorbid conditions, including severe mental illness, substance use disorders and chronic medical conditions, in addition to criminal justice involvement and housing instability. In order to meet the needs of this specific population, the role will require meeting with clients in their home once they are housed and engaging with some clients while they are still homeless. As such, it requires a high degree of flexibility to meet clients in non-traditional settings such as parks or libraries.
Under the supervision of a licensed physician, serves as a provider in a community mental health setting providing medical services within scope of license, training, credentialing, current standards of care and per quality criteria of the employer. Determines and documents evidence of psychopathology, if present, and makes a diagnostic formulation. Assesses client's needs and options to satisfy the requirements of the case. Discusses with the client his/her findings and recommendations. Decides whether initiation of treatment is urgent: medication, labs, other diagnostic tests, hospitalization, social setting, detox program; verifies that written consent for medication of client has been secured, documents therapist's plan and/or completes other referral/medical forms as appropriate. With consent of client, carries out treatment plan, documents in prescribed manner the interactions during sessions and progress of therapy. Discusses his/her opinions and/or findings with therapists and makes recommendations when indicated. Works harmoniously with Unit Manager(s), Medical Director and Chief Medical Officer to define and deliver quality clinical medical care for all clients with the units.
License(s): Current and valid Texas driver's license and a clean driving record required. Licensed as an Advanced Practice Nurse-Psychiatric Mental Health or Physician Assistant in Texas is required. Current DPS and DEA licenses.
Hours: 40 hours; Monday-Friday.
Salary: $113,678-$134,967 depending on years of licensed experience.
