Job Summary: The primary purpose of position is to provide recovery focused intensive community based services for people served by the Assisted Outpatient Treatment (AOT) program. AOT is the practice of delivering outpatient treatment under court order to adults with severe mental illness who meet specific criteria, such as a prior history of repeated hospitalizations or arrest. It is a tool for assisting those individuals most at risk for the negative consequences of not receiving treatment. This service is done by working collaboratively with an interdisciplinary team, using the ACT model, to assist consumers to foster a connection to the community and build support networks that promote wellness, while decreasing the need for inpatient hospitalization and/or incarceration. The position will require strong engagement and harm reduction intervention skills. This position will require effective collaboration with various community partners from the hospital and criminal justice field. The specialized responsibility of this position if to coordinate and monitor the holistic needs of clients served.
Hours: Monday-Friday, 8:00am-5:00pm.
Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
